A valid email address, phone number, and complete mailing address are required

A working camera and microphone are required for all appointments
Headphones are recommended for privacy and sound quality -
Your personal health information is encrypted during application submission

All sections of the application must be completed
Do not leave sections blank unless truly not applicable — this includes Social History -
Insurance information must be entered exactly as shown on your insurance card
(Member ID and Group Number — include any letters that precede numbers) -
Within 48 business hours of submission, you will receive an email with next-step instructions
