EMAIL, Phone, and Complete Address must be entered into the demographics section of the application or it will not create a medical record.

Camera & microphone are required for appointments - headphones work best.

Personal Health Information is encrypted during application transmission.

Complete all application sections. Do not leave sections blank unless there is no applicable information for that section. **This includes the Social History**.

Enter Insurance Member ID & Group number (include all letters & numbers).

Within 48-hours of application submission (business days), you will receive an email w/ additional instructions. Check spam/junk folder and call if you don't receive the "Healthier You Instructions" emails and information.

Initial Psychiatric Evaluations are scheduled for 90-minutes it's advised to allow yourself 2-hours in case of technical problems.
