Camera & microphone are required for appointments - headphones work best.

Personal Health Information is encrypted during application transmission.

Enter Insurance Member ID & Group number (include all letters & numbers).

Within 48-hours of application submission, you will receive two emails. One w/ additional instructions & another w/ questionnaires. Please check your spam/junk folder before calling. Call if you don't receive both emails and information.

Initial Psychiatric Evaluations are scheduled for 90-minutes it's advised to allow yourself 2-hours in case of technical problems.
