FEES AND INSURANCE

Accepting BCBSNM and most BCBS plans including BCBSNM Turquoise Care Medicaid.  All other insurers, please contact the clinic to verify acceptance.

SELF-PAY / OUT OF NETWORK - Please call 505-535-3033 for information on pricing.

Self-pay fees are due at the time of scheduling an appointment.  Upon request, we can provide an invoice for your insurance submission as an out-of-network provider.  Unfortunately, HYLLC has no control over how much, or even if your insurance will reimburse you.  Please direct any questions to your insurance company prior to receiving services. 

 

Copayments, deductibles, coinsurance - These payments are the patient's responsibility and are due either at the time of check-in for appointments, or upon invoice receipt from HYLLC.  Whatever portion of payment your insurance company determines is "patient responsibility" is due as part of receiving services.  Please call your insurance company prior to receiving services for any questions about your specific plan. 

 

MISSED APPOINTMENTS AND LATE CANCELLATION FEES - HYLLC requires a minimum of 24 hours advance notice for cancelling appointments. In most cases HYLLC compensates their providers for their weekly time commitments whether the patient is seen or not.  As a private business we do not receive federal or state money.  Missed appointments are not covered by insurance and are 100% patient responsibility and unless other arrangements are made, need to be paid before rescheduling the appointment. 

**Not receiving a courtesy appointment reminder is NOT a valid reason for avoiding the missed appointment fee.**  

SERVICES OUTSIDE OF APPOINTMENT TIME

Requests for completion of forms, paperwork etc. should be made during appointments (if possible) as there are additional fees for work completed in-between appointments.  These fees are not covered by insurance and the amount due is based on time.

We strongly recommend that you call your insurance company prior to scheduling an appointment in order to verify eligibility and benefits.  Please call to verify if you have an unmet deductible (usually resets each year), coinsurance and/or copayments. **Patients are responsible for paying any amount not covered by their insurance.**

 

**If you are covered by more than one insurance company, please check that BOTH companies are under contract with HYLLC as we will not be able to bill them for services.**
