Upon request, we can provide an invoice for out-of-network insurance submission

Healthier You, LLC does not control insurance reimbursement and cannot guarantee payment

Patients are responsible for contacting their insurance company before services to verify benefits
Copayments, Deductibles & Coinsurance

All copayments, deductibles, and coinsurance are the patient’s responsibility

Payment is due at check-in or upon invoice receipt

Any amount designated by your insurance as “patient responsibility” is required to receive services

Missed Appointments & Late Cancellations

A minimum of 24 hours’ notice is required to cancel or reschedule appointments

Missed appointments and late cancellations are not covered by insurance

These fees are 100% patient responsibility and must be paid before rescheduling, unless other arrangements are made
Please note: Not receiving a reminder message does not waive missed appointment fees.
Services Outside of Appointment Time

Requests for forms, letters, or paperwork should be made during scheduled appointments whenever possible

Multiple insurance plans:
If you are covered by more than one insurance carrier, both plans must be contracted with Healthier You, LLC. We are unable to bill plans that are not under contract.
